This includes preventative health management support, pre-surgical guidance and enrollment into activities that support a successful acute experience, interventional and discharge coordination including discharge medications, necessary alignment with self-care products, nutrition, and follow-up service appointments (i.e. home care, rehabilitation, primary care, medication or nutrition therapy management).
